The power elites like this chump placed themselves above the law, thinking there's no consequences, that their wealth and status placed them above the little people.

Speaking as someone who worked in health insurance, yeah, it's straight up murder and it's not uncommon to deny on first claim in the industry at all. It's the same principle as warranties and rebates - making the process so arcane and difficult that people just give up.

I admit, I wasn't a fan of SMILE. Too much reliance in jump scares and a patently false unreliable narrator trope. 2 is proving to me that the filmmakers have definitely improved. They're leaning in to the premise of the Entity is absolutely real, and we get a creepy escalation of events from that.
