Titles (like doctor, mom, officer, etc.) are only capitalized when used as names. So, for example, the emperor is Emperor Kuzco. Other examples:

The president is a rapist.
Donald Trump, the president, is a rapist.
That man, President Donald Trump, is a rapist.
