In case you needed a reminder that Strep pneumo urinary antigen has poor sensitivity. Let’s stop ordering tests that won’t change our management plan #TWDFNR #IDSKY

Step 3: antibiotic!

Using meropenem-vaborbactam for this KPC-3 Enterobacter cloacae. Started with a loading dose over 30 minutes, then doing 3-hour extended infusions every 8 hours with the syringe pump.

Yes - I feel overwhelmed in large crowds. The inoculum effect applies directly to me 😂

Step 2: set up the SEV model

This ex vivo model is a controlled environment that represents infective endocarditis within the central compartment (where those vegetations with E. cloacae are hanging out)

Learning about SEV (simulated endocardial vegetation) models! ID pharmacy residents at UK can spend a month in the PK/PD lab at the college and learn how to do bench top studies.

Step 1: make the clot, then inoculate it with organism (working with Enterobacter cloacae with KPC-3)
