for a lot of bikes, SKS Bluemels or something similar is all you need. they’re flexible enough to be easy to install, but are pretty good at staying in place.

the crank arms are fixed to the spindle with cotters (tapered pins made of soft metal).

if the cotter comes loose and falls out (which is what happened here), the crank arm can rotate freely and/or fall off the spindle.
