Community health centers provide essential care to 31M+ people—including uninsured people, people with low incomes, and people who live in rural areas.

They're already in a precarious financial position, and this abrupt funding freeze doesn't help. www.commonwealthfund.org/blog/2024/co...

Community Health Centers can't access the HHS Payment Management System today.

Section 330 grant funding is a significant portion of how FQHC/ RHC keep their doors open

I suspect (hope) that the freeze will be lifted soon for them, given their strong bipartisan support

Despite what seemed like unprecedented increases in utilization CMS's "growth rate" adjustment for 2025 was only 2.3%

Since then, the higher utilization has continued (and hospital stocks soared)- would CMS actuaries reflect this in 2026 Advance Rate Notice?

yes

The CR budget agreement had key healthcare provisions:

*telehealth extension
*2.5% increase for doc pay
*Alternative Payment Model bonus at 3.53%

Absent Congressional action, physicians will face a nearly 3% cut in Medicare reimbursement and no incentive for participating in advanced APMS in 2025

Huge respect for what @pahlkadot.bsky.social has done, and even more for what she tried and failed to accomplish, in actually improving government efficiency.

I share the fear and the conflicted hope she articulates so beautifully

Hard to really disagree w any of these predictions

If I had to differ, there may be HC legislation to fix MEI and hopefully address primary care and APMs (though nothing like repeal and replace)

idea for a book:

the Constitution is terse and purposefully spare (leaving much ambiguity for future generations to decide). But there are still parts that have never been used, were unnecessary!

I'd love to read about them, and what made the Founders err

I am having such a a strong visceral reaction to this article. Arguing that "magnificence" is more worthy than a focus on saving the most lives we can? Don't we have enough marble chiseled with donor names?
