Can't put bollards right at edge of carriageway so clearance plus width of bollard plus you lose 0.5m of effective width where there is an adjacent vertical obstruction of more than 150mm high.

You lose approx 1m of width then, though. In this case the carriageway has already been squeezed significantly, I don't see where you'd get that 1m from without compromising the footway or cycle track

The only driver who didn't give us a comfortable amount of time and space was (you'll be shocked) the driver of a black range rover with vanity plate 🙄 Everyone else - not that we saw a huge amount of traffic - was very sensible and patient with anyone walking.

The reason for this, and the one further down the road, is to protect this presumably rather ancient stone bridge from idiots trying to force their too-wide vehicle across and damaging it.

The level crossing was the issue again today, apparently! As you say, you expect the occasional hiccup but it shouldn't be the case that you can't rely on it at all.
