As a GP, I initiate conversations about limited life expectancy when appropriate. They are less initiated by secondary care teams and patients sometimes disagree with me- ‘my consultant is trying to build me up’. An important topic for discussion particularly for secondary/primary care interface.

Uncomfortable and important article by Prof Mark Taubert around what an assisted dying death actually looks like. For many it is worse than the natural death the vast majority have now. These peaceful undramatic deaths have been lost in the polarised rhetoric.
