Flagging #EULAR 2024 #Rheumatology update on immunosuppressive drugs in pregnancy, breastfeeding & guidance for men again

V. imp for many of our patients, some PILs out-of-date

TL;DR

Many csDMARDs compatible; tighter on NSAIDs/steroids; TNFi ok through pregnancy; all bDMARDs OK in breastfeeding

I still find it surreal that, almost a decade-and-a-half after leaving New Zealand, I still get emails to say that I am "owed a payment" for not using all my annual and study leave during the couple of years I worked as a doctor there...

I remember HR desperately calling me...

And of course, the pathergy reaction - where a papule or pustule forms 24–48 hours after a minimal trauma, such as a needle prick.

Smiling because the Derm Don, Ian C, is with us today at the Behçet's centre 👍🏻

Skin shenanigans in this disease can include:

- Pseudofolliculitis (acneiform rash)
- Superficial thrombophlebitis
- Erythema Nodosum

(Aphthous ulcers, which is why we have an oral med consultant)

Despite a strong field of candidates for the @rcophth.bsky.social President election, I'm backing Stephen Kaye

His commitment to being a visible, accountable leader who takes action, champions the role of the Ophthalmologist, & fights for SAS doctors & trainee opportunities is key right now

The probability of securing an ST1/CT1 post from 2018-2025 😢(Look at that change for #Psychiatry training)

Structural crisis and lack of training opportunities have created a bottleneck.

World Mental Health Day - 10th October 2025 💚

She leaves behind an immense and indelible legacy, pushing ophthalmic research nursing forward, and it was an honour and privilege to have known her.

My sincere thoughts are with all her loved ones, and with the entire CERC and St Paul’s family, during this difficult time.

Dasha meant a great deal to many people. For me personally she offered consistent support, both professionally and on a human level.

I last saw her only a few weeks ago, still working tirelessly and contributing in her own truly superhuman way, despite her illness.

I am very saddened to hear about the passing of the amazing Dasha Koneva.

As well as being a friend to many, Dasha was our inspirational Research Matron at the Clinical Eye Research Centre at St Pauls. She was exceptionally hardworking and dedicated to improving patient care and research outcomes.

I spoke on #Babinski: pithiatism, his break from Charcot’s hysteria model, & the lasting role of suggestion in functional vision loss.

Also learnt about Dr. Knock (1951): “Ça vous chatouille ou ça vous gratouille” – a French lesson in the art of suggestion! 🇫🇷🎭

Overall, #EXPO2025 was inspiring and educational, blending medicine, science, culture & imagination.

A bold ambition for the future, one we can only hope lasts.

188 pavilions! Some highlights for me:

- Osaka Healthcare Pavilion: graded my brain/eyes/teeth/skin, then “reborn” in 2050 👶
- 🌱 Natureverse: preventative health, dream pods & an iPSC-derived beating heart
- 🌊 Blue Ocean Dome: waste-free, ocean revival

The Expo felt like a tug-of-war between 2 japanese traits:

🤝 Omotenashi #おもてなし – world-famous hospitality

🎮 Otaku #オタク culture – queues full of stampbook
completionists hunting pavilion stamps

The mascot is #myakumyaku

Name means pulse/vein. A mysterious, slightly Lovecraftian creature symbolising the continuity of civilisation + interconnectedness of life, with its flowing body & “red blood cell” head.
