presents a very real risk of intense prejudice. The fear of this will no doubt cause many trans people to withdraw from elements of public life. I recall with horror how a trans friend, ill with Covid during the first wave - when mortality predictions were frighteningly high - didn't go to hospital. The fear of discrimination for being transgender was greater than the fear of this deadly new virus. Such fear among trans people is now escalating dramatically. Allyship is not a title or role that can be assumed on a personal whim. One can only be an ally by continually practising allyship. Rainbow imagery, such as badges and lanyards, is good for visibility. However, in a time when equality is sliding away from our community, symbolism alone is simply not enough. While the LGBTQ+ community has faced fairer weather in years gone by, now, as we traverse more turbulent waters, we need our advocates and allies to stand up the most. Allyship cannot be demonstrated only in fair weather. Bulletin: Summer 2025 Opinion Anaesthetists, advocacy and 'allyship' As I progress further into my anaesthetic career, I'm struck by the degree of advocacy undertaken by anaesthetists during patient journeys through healthcare. Author: Dr Duncan McGregor, Anaesthetist in Training, North-West England; Ex-Chair of GLADD - The Association of LGBTQ+ Doctors and Dentists; Honorary Assistant Professor at the University of Nottingham and an LGBTQ+ Healthcare Activist Advocating for effective analgesia in labour and perioperative care; advocating for high-quality care by considering patients as a whole; often advocating for patients for whom no intervention may be in their best interests. Much of our work and training as anaesthetists is centred on ensuring that our patients are advocated for to receive the very best possible care wherever we're involved. However, for some of us, our advocacy work can involve other areas too. I've spent nearly 10 years in the field of LGBTQ+ health activism. While perhaps a blink in the eye to some in this area, it's been long enough for me to witness progress and, subsequently, regression of rights for and experiences of people in my community. In the decade from people in my community. In the decade from 2013 to 2023 the number of reported hate crimes in the UK arising from sexuality and gender identity rose by 462% and 1,426% respectively. These are shocking statistics and paint a shameful picture for us to contend with. At the time of writing, the effects of the ruling by the UK Supreme Court reframing the protections of the Equality Act for transgender people are yet to be seen. The outcome, however, is likely to be regressive for the lives and liberties of transgender people. Subsequent, hastily released, guidance from the Equality and Human Rights Commission took a hardline approach to interpretation, recommending the exclusion of transgender people from gendered facilities such as changing rooms and toilets in workplaces and public buildings. For large numbers of transgender people, these are facilities they've used for many years. Transgender people now face a choice - to use facilities incongruent with their identity, or to be excluded from public life. For many trans people, the prospect of the implementation of this guidance, forcing them to utilise gendered facilities not in keeping with their lived gender, presents a very real risk of intense prejudice.

They aren’t even hiding it. Not for one service because of sex assigned at birth, not for the other because of presentation.
Excerpt from consultation which reads: “13.3.7 The impact on those who will be excluded from the service includes both the impact on people of the opposite biological sex generally and the particular impact on trans people of the opposite biological sex. In separate or single-sex services, a trans man will be excluded from the men-only service because his biological sex is female, and a trans woman will be excluded from the women-only service because her biological sex is male. Trans people are likely to be disadvantaged by this, by comparison to people who are not trans.” Excerpt from consultation which reads “13.5.5 For example, a trans man might be excluded from the women-only service if the service provider decides that, because he presents as a man, other service users could reasonably object to his presence, and it is a proportionate means of achieving a legitimate aim to exclude him.”

Recommendations certainly err on the side of caution, but not firmly stating mRSI is necessary in these patients.

Box 1 Recommendations for the peri-operative management of patients taking glucagon-like peptide-1 receptor agonists and glucose-dependent insulinotropic peptide. • The risk of pulmonary aspiration and mitigation strategies should be discussed with the patient using a shared decision-making approach. • Patients should continue to take glucagon-like peptide-1 receptor agonists throughout the peri-operative period. • Patients and clinicians should adhere to recommended fasting guidelines. • Upper gastrointestinal symptoms alone should not be used to determine gastric content. • Regional anaesthesia should be considered as the primary anaesthetic technique, if appropriate. • Point-of-care gastric ultrasound should be considered before induction of anaesthesia to facilitate risk stratification, if appropriate. • Individualised pulmonary aspiration risk assessment should be completed, accounting for drug, patient and procedural factors. • Anaesthesia and airway management should aim to reduce the risk of pulmonary aspiration on induction of anaesthesia, during maintenance and after emergence of anaesthesia. This could include: administering prokinetics; using a tracheal tube; modified rapid sequence intubation (with or without cricoid force, depending on local practice); head-up position for induction of anaesthesia; potential use of gastric tubes to empty the stomach before induction of anaesthesia and tracheal extubation; and awake tracheal extubation.

I think that the wording here does not inherently prohibit IV agents being used. A physician cannot administer, but could cannulate, set up and attach pumps and the person presses start.

(3) In the Suicide Act 1961, after section 2A (acts capable of encouraging or assisting suicide) insert— "2AA Assistance provided under Terminally Ill Adults (End of Life) Act 2024 (1) In sections 2(1) and 2A(1), a reference to an act that is capable of encouraging or assisting suicide or attempted suicide does not include the provision of assistance to a person to end their own life in accordance with the Terminally Ill Adults (End of Life) Act 2024. (2) It is a defence for a person charged with an offence under section 2 to prove that they - (a) reasonably believed they were acting in accordance with the Terminally Ill Adults (End of Life) Act 2024, and (b) took all reasonable precautions and exercised all due diligence to avoid the commission of the offence." 25 Civil liability for providing assistance (1) Providing assistance to a person to end their own life in accordance with this Act does not give rise to any civil liability.

At this point, the co-ordinating doctor or an authorised alternative doctor can administer drugs for the person to self administer, to support the patient in self administering or to administer directly to the person. The details of which drugs will be determined by the Secretary of State. 10/12

(5) The coordinating doctor may be accompanied by such other health professionals as the coordinating doctor thinks necessary. (6) In respect of an approved substance which is provided to the person under subsection (2), the coordinating doctor may - (a) prepare that substance for self-administration by that person, (b) prepare a medical device which will enable that person to self-administer the substance, and (c) assist that person to ingest or otherwise self-administer the substance. (7) But the decision to self-administer the approved substance and the final act of doing so must be taken by the person to whom the substance has been provided.

There is provision for the use of a proxy to sign all documents in the case that a person is physically unable to sign - capacity rules must still be met. 8/12

(4) A proxy may not sign a declaration - (a) unless satisfied that the person understands the nature and effect of the making of the declaration, (b) if disqualified under section 36 from being a proxy, or (c) if it is a second declaration and the proxy signed the first declaration as a witness. (5) In this section "proxy" means - Terminally Ill Adults (End of Life) Bill 11 (a) a person who has known the person making the declaration personally for at least 2 years, or (b) a person who is of good standing in the community.

Importantly here the Bill specifies that this must be undertaken by a Registered Medical Practitioner, I.e. a doctor.
The specifics of what experience and qualifications are required will be determined by the Secretary of State. This is the case for both assessment and administration. 6/12

The process is that there are two independent doctors undertake separate assessments of:
- the nature of the illness
- the person’s capacity
- has a clear intention to end their life
- that the decision is free from coercion

(3) If, having carried out the first assessment, the coordinating doctor is satisfied that the requirements of subsection (2)(a) to (g) are satisfied, the coordinating doctor must— (a) make a statement to that effect in the form set out in Schedule 2, and sign and date it, (b) provide the person who was assessed with a copy of the statement, and (c) refer that person, as soon as practicable, to another registered medical practitioner who meets the requirements of section 8(6) and is able and willing to carry out the second assessment (*the independent doctor"). 8 Second doctor's assessment (independent doctor) (1) Where a referral is made under section 7(3)(c), the independent doctor must carry out the second assessment of the person as soon as reasonably practicable after the first period for reflection has ended. (2) "The second assessment" is an assessment to ascertain whether, in the opinion of the independent doctor, the person who made the first declaration - (a) is terminally ill, (b) has capacity to make the decision to end their own life, (c) was aged 18 years or over at the time the first declaration was made, (d) has a clear, settled and informed wish to end their own life, and

There is no obligation for a medical practitioner to raise the subject or to undertake a discussion if raised by a patient. However, they must refer on if requested.

(4) If a registered medical practitioner conducts such a preliminary discussion with a person, the practitioner must explain to and discuss with that person - (a) the person's diagnosis and prognosis; (b) any treatment available and the likely effect of it; (c) any available palliative, hospice or other care, including symptom management and psychological support. (5) A registered medical practitioner who is unwilling or unable to conduct the preliminary discussion mentioned under subsection (3) must, if requested by the person to do so, refer them to another registered medical practitioner whom the first practitioner believes is willing and able to conduct that discussion.
