Most clinicians will say someone who falls from standing and breaks a hip has a traumatic fracture.

Impact = trauma.
Simple.

Except that's not really the way to look at it.
At least for coding (but even orthopedics agree).

And it seemed likely jibberish at first glance.

Some physicians try to document with the fewest words possible...

Many think you don't have to know billing anymore because ambient AI scribes are doing it all for you.

Recently I had to talk to a surgeon about how his documentation did not satisfy the 99214 he dropped.

The problem?

🚨 Academic attendings suck at teaching billing (sorry).

So residents graduate not knowing what they're doing.
But your real job assumes you do.

So you rarely bill a high level to "fly under the radar."

"Some company came in and told me I was over billing" a surgeon told me.

He wasn't.

They came to that conclusion based on a small sample of charts.

I looked at his overall billing patterns.
I looked at this notes.
I discussed his cases with him.

He wasn't overbilling.
He was under documenting.
