Yes, concise notes are often better because they make important information easy to find.

But they must also be clear (and complete).

You do not need to write a novel.
You just need to write enough to show what you were thinking and why.

That doesn't result in any true time efficiency gains (esp as your time is going to be spent clarifying your notes - as was required in this example)

❌ This is not a winning strategy.
❌ It's not good for patients.
❌ It's not good for your own coding / RVUs.
❌ It's not good legally.

was making sure to verbalize the buzzwords to be picked up by the audio.

But that wasn't happening.

The ambient AI scribe was changing the words - resulting in a downcode.

This is a perfect example how this new tech could be secretly costing you precious RVUs

He says he clearly discussed the options of a major surgery and he/the patient decided not to pursue it.

We had met with him before so he knew what to include in his note.

I spent 1 hour with him and he is now documenting less, collecting more RVUs, billing more consistently with national averages, and his notes are audit proof.

I can't spend 1:1 time with you, but…

His benchmarking data actually showed he was dropping way more 99212s than national averages (and way less 4's and 5's).

And in discussing what was actually done during his clinic visits, he wasn't using the right language in his notes.
