IMO, hard to evaluate form from this angle. Only thing I can add is neck position. Pick a spot on the floor ~5 ft in front of you and look there the entire lift - keeping a neutral neck/spine. Looking at the mirror while raising/lowering can strain your neck/back at heavier weights.
