Interesting. You can see how it was derived from the original theatrical poster by Tom Jung (below), though his work is far less crude.
Maybe writer/director Ed Wood — or Jung, himself — held the art rights and wouldn't let the 8mm distributor use it, or this was a way around any licensing fee.

I've never been one of those who feel a need to anthropomorphise Death as some sort of oi immortal, otherworldly being who silently traipses about our planet wielding a huge, ghastly scythe to collect his grim harvest.
But if he is, you can bet Kissinger has signed a contract with that bastard.
