It's mostly in the drawn NSFW content, I've found, which is a lot harder to catch via algorithm. In contrast, real life NSFW gets too many false positives.

Except people have to actually use them. There are so few creators that use the tags, it's impossible to follow them and keep a feed friendly for anyone stealing a glance in public.
