Page 5. There goes that supposed $500 million in savings. There’s more bad news on the cost front. New York employs Managed Long Term Care (MLTC) insurance companies who determine the care people get. In written testimony submitted for the August 21 hearing, the MLTC association stated that PPL is demanding payments for customer care that are “often higher” than what they previously paid. Other testimony at the hearing said this will result in over $500 million in reduced profits for the MLTCs, who will inevitably demand more funding from the state. There’s a photo of Caring Majority advocates holding signs that say, “CDPAP workers are essential,” Home Care heroes not insurance profits.” “Stop Insurance Greed! Raises for home care heroes, not insurance CEOs!” Page 6. If New York truly wants to save money while providing fair home care, the profit-based system must be scaled back or eliminated. End the PPL monopoly and bring back the responsible nonprofits that previously ran CDPAP. For even more savings, the Home Care Savings & Reinvestment Act eliminates profit-grabbing middlemen — the MLTC insurance companies that raked in $2.4 billion in profits from 2021-24. Underneath there’s a diagram flow chart showing money being handed from New York State to Private Insurance companies (and then the money flows outwards towards the words “billions pocketed by private insurance companies”). Then, the remaining money goes to home care agencies & fiscal intermediaries, and the remaining money that’s left goes to home care for older & disabled New Yorkers Page 7. Let New York become an example to the rest of the country, by treating healthcare as a public good, not an opportunity for obscene private equity profiteering. According to the industry publication Mergers & Inquisitions, partners in private equity earn, on average, from $1 million to $2 million per year. The CEO of PPL, private equity partner Miki Kapoor, bought his home in Westchester County for $2.3 million in 2013. There are two images. The other is a screenshot from the hearing of the DOH Commissioner MacDonald, and there is a caption of him saying, “I have no…no way to know that.” The beginning of the article says: “A turbulent hearing held by the State Senate in August examined mismanagement of the state’s Consumer Directed Personal Assistance Program (CDPAP). A private equity-backed corporation, PPL, was brought in early this year to run the popular home care program, replacing hundreds of smaller companies.” Page 2. “But the transition to PPL has been plagued by problems. In a recent survey, 55% of CDPAP workers were not fully paid and 20% of consumers had gone without approved services. There are two lawsuits pending over wage theft. At the hearing, Department of Health Commissioner Dr. James McDonald said the key reason for changing CDPAP was “lowering administrative costs,” claiming the state would save $500 million a year. Yet when asked about those savings, or his department’s supposed oversight, he could not give any answers.” Photos of home care consumers who use wheelchairs and home care workers holding signs that say, “Save CDPAP NOW!” “250,000 Workers Unpaid,” PPL Doesn’t Pay” “Hochul! Our Lives are on the line” Page 3. When CDPAP began 30 years ago, disabled people designed a program that gave them control over who comes into their home to help with intimate tasks. By allowing relatives to become paid caregivers, CDPAP addressed the growing need for home care while helping solve both the shortage of home care workers and the desire of families to give their disabled loved ones care without going broke. It also cost the state less than the home care agencies – New York’s other option for Medicaid-funded care – by cutting administrative overhead because consumers (or their representatives) could direct their own care.” Below there is a photo with the caption “Defending the hard-won CDPAP program (Caring Majority Rising, Albany, 2025)” The photo is of home care consumers blocking escalators in the capital with their wheelchairs, one with her fist in the air. In the foreground, there is a capital police officer. Page 4. Not surprisingly, CDPAP grew in popularity. But instead of celebrating its success in meeting an essential need, the Governor saw an opportunity to make cuts, citing “waste, fraud and abuse” that barely exists as a justification. Enter PPL, a corporation designed to profit off publicly-funded home care. Is PPL, in fact, saving the state taxpayers money? The early evidence is an emphatic no. PPL’s incompetent handling of the transition has led to roughly 80,000 CDPAP consumers fleeing the program, most to sign up with licensed home care agencies. According to testimony at the hearing, based on the Department of Health’s own numbers, that switch will cost taxpayers $550 million more per year. "The Titanic is sinking, and the captains are abandoning ship. The collapse of PPL's executive leadership tells you everything you need to know about the current state of the PPL transition. When a company's CEO, CFO, and President all exit within weeks of each other, that's not a planned turnover - that's a company in freefall. This should be the final wake-up call for Gov. Hochul and DOH Commissioner McDonald to finally terminate this failed contract and restore the trusted local providers who successfully managed this program for decades. This leadership exodus demands answers: were these officials fired for PPL's catastrophic performance in New York? Did they resign rather than face accountability for the crisis they’ve created? The Senate's August 21 hearing must extract these answers from PPL - regardless of who may be left to answer them. Even more alarming is who’s taking control. PPL will now be run directly by the head of Linden Capital, a private equity firm that owns part of the company - a clear signal that cost cutting will drive even more decisions going forward. New Yorkers should brace for more corners cut, more workers left unpaid, and more consumers left without quality care. But no amount of leadership change can fix PPL’s fundamental incompetence. PPL has proven it cannot deliver basic services like paying workers or enrolling consumers - because they only care about the extraction of profits, not providing quality care. That will never change, regardless of who sits in the C-suite. It’s time to hit reverse on this failed contract and finally start demanding real accountability from this train wreck of a company. Every day we delay means more suffering for the hundreds of thousands of New Yorkers PPL continues to abandon."
