Great thread- I’ll be reconsidering my practice.

[Why] is furosemide susceptible to malabsorption from "gut edema"?

When a patient with heart failure is hospitalized with congestion I often hear "let's use IV furosemide; they're probably not absorbing the PO."

It's a comment unique to furosemide.

But is it accurate?

What I love about echocardiography in critical care is how you can see physiology unfolding before your eyes. This is a case that has stayed with me and shaped how I think about treating acute LVOT obstruction #cccsky #echofirst #pocus #emimcc
A case both for not forgetting basic physical examination including auscultation and for echo in shock. Shared with patients consent. 🧵 #foamcc #foamed

I honestly believe all LLM results should come with a disclaimer reminding people that the thing doesn’t (and absolutely cannot) know any facts or do any reasoning; it is simply designed to *sound* like it knows facts and frequently sweeps some up accidentally in the process.
I really think many if not most of the LLM fail cases are people trying to use it as a search engine. It is the opposite of a search engine.

two groups we should try to get on MedSky:

(#1) people stuck on twitter (Stockholm synd?)

(#2) people who quit FOAMed & social media entirely

#1 will come sooner or later

IRL, I see a LOT of #2. these are harder to reach. we need to advocate IRL (eg conferences) that FOAMed is back & it's here.

Another useful hint to a significant acute mitral regurgitation when it can be hard to see on colour doppler. #emimcc #pocus #echosky
Here's a little visual #POCUS trick: When the mitral valve is opening a lot (low EPSS) and the LV is hyperdynamic, but the aortic valve is not opening that much, something is wrong (loop at 50% speed). #FOAMus #FOAMed #FOAMcc

++ fodder for discussion. As many have said—we will need to take next steps wisely. Other studies have given us somewhat similar conclusions re: risk of slow correction. Still—ODM is devastating; clearly we need more research to better understand this problem. evidence.nejm.org/doi/10.1056/...

This feeling is visceral.
"One large black coffee to go, please."

'Would you like any room for cream?"

"No, thank you."

"Okay! I'll just fill this 100% up to the brim so that the slightest touch will splash and scald you."

Broadly, for a clinical manuscript you have 3500 words to play with (exlcuding abstract, legends and references). These need to be distributed across the introduction, methods, results and discussion.

The distribution should look something like the formatting formula below
