What makes you so sure none of this gets passed by Congress? They’ll be able to get the Medicaid bill through as an attachment on FY26 federal spending bill and via budget reconciliation. This is effectively the end of Obamacare/ACA coverage for gender related care.

Yes, they want normalcy, not revolution.

When trans advocates start incorporating this empirical evidence into their strategies, they will have less disappointment and unmet expectations.

For the generation that came of age under WPATH, this will feel like the rug getting pulled out from under one’s feet. But it happens every 15 or 20 years. The community isn’t ready for DSM-VI or WPATH9.

Trans community got it in its head that trans kids were ‘ours’, and it was appropriate to outreach to other people’s children. DQSH, queer sex-ed in schools, suicide hotlines across state lines, pediatric medicine referrals. Fatal error that the community will pay for for decades.

Yes, fascists are setting boundaries. And the community will have a difficult few years adjusting to it. And the people who have been transgressing boundaries will be precisely the most to loose and most resistant to acknowledging any wrongdoing.

Anybody who sets any rules or boundaries will be seen as fascists. So it’s become something of an empty accusation. Want trains to run on time? Fascists and ableist for making people be on time. FDA quality control on food and drugs? Fascists against DIY cooks.

Perhaps it will pop the ‘gender affirming care’ echo chamber, and people will start critically examining the framing they’re interpreting things through.

We had similar language during HBGIDA era; so I’m not overly worried about it. But it does move transition from ‘easy’ to ‘hard’ mode.

Those unintended consequences have put the entire endeavor in jeopardy; which is why the community is in the situation it is in. Some amount of gatekeeping and friction needs to return. And while I don’t personally recommend ‘sex-denying’ language, I think it’s important for people to hear.

You’re not wrong that framing will lead to many people exiting care; and finding practitioners to work with will become more difficult. But open access where any non-specialist can refer, prescribe, or treat per insurance rules, has had all sorts of unintended consequences.

Sex-rejecting as a framework is perfectly coherent, but would lead to policy positions like ‘yeah, there are downsides to transition like social ostracization and sterility, and it shouldn’t be recommended; but if you insist, I will try to make sure you are safe’.

Having started during HBGIDA era, there are obvious differences in framing, which leads to values, which lead to policy approaches. That you don’t see any distinction between the two is due to the libertarian transgenderism framing. The do no harm transsexualism framing sees plenty of distinction.

The dude is from Texas; and the entire state is on hardcore difficult mode for trans folk. He would get 24/7 attack ads for voting yes. So, if he wants to protect adult access to GAC and immigrant issues, pediatrics is a non-starter.

As a clinician and having worked for CMS… my advice is don’t be surprised if many of those orgs pivot and comply with new regs.

What ‘trans youth healthcare’ looks like is significant up for debate based on GLAADS roundup of statements from medical orgs. Insurance coverage for talk therapy and ‘wait and see’ is just as valid to many of those orgs as HRT and puberty blockers.
