Think of a meniscus tear as analogous to when you slice your finger horizontally. When you rub the tear "with the grain" you feel nothing. However, when you rub the tear "against the grain", OUCH! The position of the knee joint determines "with" or "against" and it doesn't take much to go either way

Not sure about the NFL Rules Book, but at the lower levels of football (College and NFHS) there are LEGAL blocks in the back. They involve players in the trenches and the play at or close to the snap.
