A 60yr old comes in with acute onset drowsiness,left 7th,9th,10th and 12th cranial nerve palsy. He has a history of weight loss,hematemesis,bicytopenia(anemia with thrombocytopenia). You order an imaging and see this.

And the history is 43yrs/M history of chronic alcohol abuse-30yrs subacute onset symmetric LMN(atrophy+hypotonia+fx in medial thigh+areflexia-generalised-except triceps) quadriparesis(LL>UL;proximal>distal-LL and distal>proximal in UL) with hyperalgesia for 5 months.l While testing CN this was seen.

It occurs even on infraorbital nerve stimulation!
That's what it is present on both sides and consistently! I will share another where can be seen even on infraorbital stimulation. But I have never seen this before. The history is interesting too!
