🧵Prone positioning saves lives in severe #ARDS but it's hard to measure how often it actually happens. In 2019, we had to resort to surveying hospitals just to see if it was even an option (pre-covid, it wasn't, at most MA hospitals)...

Among those with MV >96 hrs: tracheostomies⬇️, DNR rates, palliative care, and hospice use all⬆️. Despite limitations in care, 1-yr mortality and institution-free days 1 yr after ICU admission remained constant.

We also found that patients spend very few DAOI after trach/PEG/both. Patients receiving a trach spend a median 3 days out of an institution, while patients receiving a trach and a PEG spend 0 days out of an institution within the first 90 days.

We found that short-term mortality is very high in pts receiving a trach/PEG/both. Although in-hospital mortality is relatively low (10-17%), most of these patients are sent to SNF/LTCHs, where mortality is very high. Over 40% are dead within 90 days of trach/PEG/both

How come we are this far into the pandemic and it still takes 24-48 hrs for an outpatient #covid test to turnaround??

[2kids + 1 parent]x 2 days of missed work/school waiting for 1 covid test = so much unnecessary lost productivity
