In PA, it’s HealthChoices for managed physical health, & Community HealthChoices (CHC) for managed long-term services and supports (MLTSS) - pays for nursing home & home care provided by Personal Care Assistants, distinct from (mostly Medicare) home health (nursing + home health aides)
