Thrombosis Canada: "If used subcutaneously, UFH dosed at 333 units/kg SC for the initial dose and then 250 units/kg SC twice daily is an alternative that does not require aPTT monitoring."

Copied and pasted screenshot of lexicomp dosing recommendation...suggests the same.

To put the finishing touch on this discussion...even more data on this... Table 1 is quite nice, response diff between 3.375 g IV q6H and 4.5 g IV q6H are quite similar. Table 2...lorente 2009 also shows NSS for clinical cure.

How about the data on PTZ 3.375 g IV q6h vs 4.5 g IV q6h? (which I find interesting!)

Gill & Nicolau 2023:

- 3.375g Q6H over 0.5h had adequate PTA at MIC 8 mg/L

- 4.5g Q6H over 3h or 4.5g Q8H over 4h = >90% PTA at MIC 16 mg/L

Where does the PTZ 4.5g IV q6h dosing come from tho?

This study used PTZ 4.5 g IV q6h & it found "similar number of mean hospital days, readmission rates, and frequency of AEs vs imipenem"

In fact, mirtazapine has fewer reported w/d symptoms in the data...further supporting stopping it cold turkey in these pts🚫

but, be mindful of potential reporting bias!

if dose is >30mg/day, w/d becomes more relevant since it's more adrenergic at higher doses

4/ 🚨 Giving glucose before thiamine can theoretically worsen Wernicke’s. Thiamine-dependent enzymes needed for glucose metabolism. Without thiamine, glucose breakdown might stall, causing less ATP synthesis & rapid buildup of lactic acid.
