I continue to be shocked by how PCN allergies are misunderstood

With PCN allergy, you can absolutely use cefepime, piptazo, or meropenem

Ceftriaxone, ceftazidime, cefepime, mero, all safe b’c they lack or have dissimilar R chains! Agreed, aztreonam-vanc and cipro-clinda are horrible combos to use just b/c avoiding this “cross-sensitivity”

Thnx for this Josh! It still amazes me how references still suggest avoiding BL Abxs d/t this misconception & misunderstanding. IgE-mediated allergic rxn to PEN is b/c of R-side chain. Adding caveat, cannot use BL if TENS/SJS since immune-mediated
