Okay, it might be because he's a PA. But there is a DO or MD that he works under. I would ask if they would do it, if your PCP says no (although, like I said, they should not).

Even with a stair lift in my home, I am terrified that I won't be able to escape our house during a fire due to my limited mobility. If I was able to somehow bust our upstairs bedroom window and jump out of it, I am almost certain that jump would be fatal. I don't have the strength to handle impact.

Also make sure the letter meets any specific legal requirements specified in the law. Lastly, the letter should include your diagnosis, a diagnosis code, and an approximate date that you became disabled, if your provider has the records to back that date up. 3/3

if he is willing to sign it, make sure the letter includes: the date, his official signature block with his full name and any credentials ( letters after his name ), his actual signature in pen , and his NPI number. most providers have an official stamp with their name, credentials, and NPI. 2/2.
