A palliative care team replaced email handoffs with EMR-integrated I-PASS. Within six months, utilization exceeded 99%, with clinicians reporting higher accuracy, shorter length, and less time required. I-PASS was strongly endorsed as an effective handoff tool.

Between 2016–2021, opioid doses prescribed by oncologists for cancer pain dropped five-fold, despite guideline exemptions. Long acting decreased the most.Higher pain and CAGE-AID scores predicted higher doses; non-English speakers received lower doses.

During the study period, we observed a five-fold decline in opioid dose prescribed by oncologists for cancer pain. This raises concerns for undertreatment of pain in patients with cancer.
