Mid-40’s male w/nasal obstruction

Dx: allergic fungal sinusitis (polyps not shown)

📸 layering of mucin, cellular debris, degranulated eosinophils & inflammatory cells
📸 Charcot-Leyden crystals (arrows)

This fungal yeast was called Histoplasma on biopsy. This diagnosis was later confirmed by cultures. How often does this approach go wrong?

This fungus looked exactly like Aspergillus but turned out to be Pseudallescheria (by microbiologic cultures). How should such cases be worded?
