To everyone in a similar scenario: the tactic my doctor's office has taught me is to ask, in writing, for:
1) the name, board specialty, and license number of the doctor making the determination the treatment was not medically necessary;

People who remember the 1950s: were phone books a thing then? It seems like watching movies and TV I can't remember people mentioning them before stuff from the 70s. Was it just a cultural blind spot for a while, or were universal directories not a thing?

Just got word that, at long last, I will be teaching "Women in U.S. History" this fall.
