after further investigation:
- the 1 mV step remains unexplained, but I suspect ground current issues
- the 100x gain causes the amplifier to slow down by a lot
- the 400 ns comparator delay was caused by the large 32 mV hysteresis I seem to have forgotten about
there are a few things wrong here:
- channel 4's waveform has an unexplained 1 mV step before the slope, theoretically it should be ~300 μV
- channel 2's waveform should be exactly 100x channel 4's waveform (so they should overlap)
- channel 3's waveform should happen sooner (by ~400 ns)

does this explain why when asking for help with programming, people get flamed and end up start a tribal war amongst the "experts", who then devolve into personal name-calling, but when asking for help with electronics, 3 EEs civilly discuss ideas and present one unified solution
A key factor for making technical discussions more successful (and enjoyable) is to resist the urge to one up each other. Don't try to prove you're smarter than others, instead work collaboratively towards a solution, benefiting everyone involved.
