Our final briefing of 2025 covers updates in immigration, data security, and medical research funding, plus brand-new material in trans healthcare. It’s unusually long, but we found some good news to mix in with the bad.

We banged our heads against the true costs of healthcare cuts this week and JFC. It's not just ACA price hikes + people losing Medicaid coverage, though that's already bad It's also hundreds of hospitals and nursing homes poised to fall like dominos. Healthcare will be worse for all of us.
Congressional Democrats are refusing to authorize short-term measures to keep the government open unless Republicans agree to renew ACA subsidies, and possibly to reverse Medicaid cuts as well. As the shutdown enters its third week, we look at the ripple effects of this summer’s healthcare cuts.

The clear story in Immigration this week is the escalation of—and pushback against—the Trump administration’s militarized raids and warrantless stops in Democrat-run major cities. We’ve updated our timeline with a lot of events in this zone:

here’s the clip of someone saying “you’re an idiot,” as Levi — one of RFK Jr’s handpicked vaccine advisers — suggests other scientists are being overconfident about vaccine safety data

Did they really make the CDC put time into making a presentation about a twenty year old observational study using a different vaccine? #ACIP

The Pitt has plans to tackle stories next season related to federal health care cuts. I've got some ideas in a new column out in @jama.com.

What if, instead of knowledge-shaped products… knowledge?
it occurred to me while I was in an especially gruesome mood last week that @unbreaking.org is essentially doing what AI summaries claim to be doing, just with rigor and real citations and as much accuracy as a bunch of devoted people can achieve, but make it human volunteers 🫠
