Got my pneumococcal and second shingles vaccines today and I'm feeling a bit hit-by-truckish this evening. Worth it though. Eat shit, RFK

If one must spend their Friday evening creating pneumococcal vaccines slides, then one should also put on OG @toolmusic.bsky.social on their speakers and rock out.

I got the pneumococcal as well today and no side effects. The second shingles shot was bad last year but I was thrilled to get it (brand new study shows increased dementia risk after a second shingles infection!)

Yep and flu and shingles and pneumococcal. It’s across all adult vaccinations not just Covid specifically.

If 1% of ED LPs end up being for Pneumo, and 1% of Pneumo in adults is R, you will do 10,000 vanco courses for every 1 patient with CTX-R pneumococcal meningitis. That will definitely cause a large amount of AKIs and VREs.
In LA, we recommend NOT adding vanco until LP CSF WBC/% PMN known.

Pneumococcal vaccination under age 65 is based on risk factors. I personally have had it and haven't had pneumonia since, whereas previously I had pneumonia every year or two, and I have to say I'm a big fan, but it's up to you and your doctor.

PP in the study is Pneumococcal Pneumonia
ACP is All Cause Pneumonia

The study was to see if an immune response resulted in actually immunity. The trad def of vaccine required it to produce immunity. The new (2020 definition) of vaccine requires only an immune response.

Every year in Europe:
• Flu causes millions of infections & tens of thousands of deaths
• RSV leads to ~250 000 hospitalisations in children & 160 000 in older adults
• SARS-CoV-2 continues to circulate year-round
• Pneumococcal pneumonia remains a major cause of hospitalisation among older adults

I had whopping cough when I was 7. No whop sound but correct diagnosis and such hard and heavy coughing I blew a hole in my left lung. That was followed the next year with emphysema.
Now there are vaccines for whopping cough and the pneumococcal vaccine, so no kid has to go through what I did.

Metastatic melanoma: Similar benefit was observed (HR 0.37; p = 0.0048) and longer progression-free survival.
No benefit was seen with influenza or pneumococcal vaccination, implying the effect is unique to the mRNA platform.

The cause of PP is not known and may not be any pneumococcal strain not included in the vaccines.

In short, people who got an mRNA vaccine within 100 days of starting their immunotherapy lived for nearly twice as long in this cohort, and had nearly double the likelihood of living 3 years. This effect was restricted to mRNA COVID-19 vaccines. The effect of Pfizer vs. Moderna was similar.

Shingles vaccine associated with good things in older age, when compared to people who had pneumococcal vaccine (so not just about vaccine seeking behaviour).
I'll look forward to seeing the paper when it comes out.
💥The shingles vaccine might be one of the most underrated health moves after 50.💥

New large study(174K pts):
▪️ 50% lower risk of vascular dementia
▪️ 27% fewer blood clots
▪️ 25% lower risk of heart attack or stroke

Relative to pneumococcal vaccination, shingles vaccination was tied to a lower risk of vascular dementia (50%), blood clots (27%), heart attack or stroke (25%), and death (21%). The study authors said that the findings suggest the shingles vaccine can prevent both infection and its complications.

This news was very sad to hear.
Pneumonia from any cause is very serious in older individuals. This is why we recommend pneumococcal vaccines for everyone 60 and over and RSV, Flu and COVID vaccines to prevent viral causes of Pneumonia.

Hi Brad, I’d like to hear your opinion on the optimal antibiotic approach for pneumococcal pneumonia with bacteremia — do you favor combination therapy (β-lactam plus macrolide) or β-lactam monotherapy in these cases?
October 16, 2025 at 8:51 PM
Yeah. I have auto immune disease (and get strong reactions to vaxes) and already have regular covid and flu vax and this year had whooping cough etc vax too. Next year baby grandson goes to childcare so I'll get RSV vax then pneumococcal vax then Shingles. Over time.

The Pneumococcal vaccine has also been linked to reduced dementia outcomes. It’s part of the schedule and almost lifelong

While many things can cause pneumonia, this one goes for the most likely suspects
