Headed in for an open-heart tune-up: a couple ASDs and a PAPVR are making my right ventricle overwork. Not an emergency — just preventative repairs. Going old-school chest-crack with a steel-wire reassembly. A few slow weeks ahead… wish me luck.

One of my #chronicdiseases is AdultCHD I have #papvr or #scimitarsyndrome I've since had it repaired my 1st open heart was 2015,2nd 2016,3rd 2017 Congenital Heart Defect patients need lifetime care In 2022 I needed a stent due to #stenosis in one of the repaired pulmonary veins #spreadawareness

L-sided PAPVR is uncommon. In this scenario, the L pulmonary veins often drain into an anomalous vein that drains into the innominate vein. If a LIJ catheter courses down and into the L or R lung hilum, a high degree of suspicion should exist for PAPVR

The catheter is ending in an anomalous left superior pulmonary vein. Partial anomalous pulmonary venous return (PAPVR) may result in blood draining into any of the central veins. It is present in ~0.5% of individuals & can be associated with atrial septal defects

I have papvr (partial anomalous pulm vein return/scimitar synd) so one pulm vein just goes rogue and goes to the wrong place, in mine back to IVC. The fun part is the hypoplastic lung RLL causing right shift of my ❤️ = XR looks backwards!

However, the PACs are not 100% clear and the argument for LPF PVCs is quite reasonable. Here is another ECG from the same day showing a similar pattern. This patient has normal LV function with a mildly dilated RV (and normal RV function). Prior remote ASD and PAPVR repairs.
