TL;DR: the Enterococci are really good at evolving resistance to antibiotics in new and creative ways.
Resistance to last-resort antibiotics in enterococci
Abstract. The genus Enterococcus comprises a diverse group of species, many of which are commensal members of the gut microbiota of humans and animals. The

#SewageScandal: “the water quality falls below the acceptable level for safe bathing, meaning it contains too large a quantity of bacteria such as intestinal enterococci and E coli. These bacteria can cause serious illness, so people are directed to avoid swimming in areas rated poor.”

Managing an outbreak of vancomycin-resistant enterococci
A review of the current literature supporting health professionals in managing a vancomycin-resistant enterococci outbreak found six areas to consider Abstract Vancomycin-resistant enterococci are a common cause of healthcare-associated infection. A literature review was conducted to understand the current evidence and support nurses and other health professionals tackling vancomycin-resistant enterococci outbreaks. It identified six key themes: risk factors, hand hygiene, environment cleaning, screening, antimicrobial stewardship and patient isolation.

The ability to study the genetic underpinnings of biofilms formed by enterococci & other bacteria has grown, which improves understanding of their form & function. Biliary cultures got into official recommendations (without strong evidence) so they're often done. They tend to show enterococci (among others) so patients then get add'l AB coverage.
Nice study showing there is no discernible added value to doing those cultures/widening coverage. #IDSky #MedSky
