my ocs eden and valentine. valentine has a stick of pocky in his mouth in the first panel and eden licks him with his big nasty dragon tongue with his glamour off in the second. the pocky stick doesn't even get eaten my ocs damares and moros. moros has the pocky stick sticking out of his helmet. in the second panel damares has his hood all the way over moros's helmet and he's choking him. damares says " i know you were going to suck it back into your helmet, jerk" and moros replies with "oooh, kill me about it, then." damares says "i'm going to smear your blood and CSF aaalll over" and moros says "yaaaayyy." there are hearts around the dialog cuz this is how they flirt with each other as undead who have become addicted to dying lol

If 1% of ED LPs end up being for Pneumo, and 1% of Pneumo in adults is R, you will do 10,000 vanco courses for every 1 patient with CTX-R pneumococcal meningitis. That will definitely cause a large amount of AKIs and VREs.
In LA, we recommend NOT adding vanco until LP CSF WBC/% PMN known.

Yup, posted on the other platform. For PK, 2 gm CTX Cmax blood 250 mcg/ml, 10% CSF = 25 at peak. 4-6 hr half life:
T0 = 25
T4 = 12.5
T8 = 6.25
T12 = 3.125 --should be above MIC whole interval.

As far as the massive overuse of vanco, does depend on geography. In much of US, CTX-R rare in adults...

brains are cool, my scan vs a normal MRI
MRI, sagittal view, T1, cropped to show my cerebellum and brain stem. The bottom of the cerebellum has descended down and now crowds my brain stem and spinal cord. A normal MRI I found on google - the cerebellum is higher, inside the skull, and there is more black space (CSF)

This is something we have seen in a lot of people who have this condition and it’s comorbidities for a while, a lot of us have signs it’s there but there’s a specific incident that causes symptoms to increase. For me it was my csf leak and the sealing of the leak.
