Even the above pyramid overplays metaanalysis /systemic reviews, because it depends entirely on what is being reviewed and the study design / weightings / (in some cases, outright "hot takes") of the SR. E.g. a crappy metastudy of cohort studies pales in relevance to a single good RCT.

i read a metaanalysis on porn addiction studies and indeed there is no solid ground to describe and diagnose porn addiction, since addiction is a severe narcological-psychiatric disorder. BUT there is comorbidity with unhealthy porn consumption and having a psychiatric disorder. OCD for example.

i have yet to see a single metaanalysis that supports the serotonin theory of depression but "you're depressed because of a chemical imbalance" is a perfect thought-terminating cliche for people who want to invoke the language of science without understanding it.

Amazing. Someone points out that the results of nudge metaanalysis are actually all publication bias. Original authors then brush it off with ”[being] a relatively young field … comes with some methodological growing pains.”
there have been big questions raised about the quality and reliability of the research suggesting that small nudges (like moving an entry field or asking people to tick a box) meaningfully affect behaviour

Stakeholders need to be involved in setting practice guidelines. They can't be with an opaque model doing metaanalysis. Models do not currently do RoB and pooling, but are expected to be able to in the future. They'll need access to everything, not just the OA subset, but could work with local PDFs.
