Like so many #addiction treatment providers, I once saw mandated (i.e. forced) treatment as a "necessary evil."

I don't know when I first internalized the idea that evil is ever "necessary," but I'm working to unlearn it now.

We need to get comfortable with being wrong and also support colleagues as they realize they are wrong. If we're really here for our patients/clients/participants/people, rather than status or ego, we are willing to change our minds and approaches.

People who reject #HarmReduction are uncomfortable with the increasing number of experts who they thought were on-side, who looked at the evidence and shift to support harm reduction.

Do you never consider the remote possibility that you might be wrong? And the harm you cause if you ARE wrong?

Real care does not scale. As long as we keep focusing on "efficiency" or "streamlining" we are not providing real care.

- we tell patients and colleagues- “do NOT stop bupe if surgery or acute pain, full agonists work with adequate dosing”
- we tell patients “taking bupe provides overdose protection if you use fentanyl”

- Requiring PA for bupe = absurd
- Requiring "comprehensive treatment" = absurd
- Requriing XWAIVER number (hasn't existed x years) on form = absurd
