Yes when everything is done 100% according to the book, everybody is knowledgeable, up to speed and all quality gates are met than yes. But reality is messy and in the end still people make choices. Things can go south also with the big guys.

Large systems, organizations and life is complex, you can't prevent everything. Accept and improve.
