Before the ACA option (which I have not used yet but was always glad it was there as a choice), when I was self-insuring for years, I wouldn't seek treatment for things like migraines because having that on my record was grounds for denial of insurance coverage for that pre-existing condition.

It is why I have the job I have now - for insurance when spouse retired and we lose theirs. I hate that insurance traps us.
