Similar in a way to how Filianore isolated the Ringed City's time, Placidusaxx split some land with his power, creating the Lands Between, becoming an isolated world. Beings from beyond the fog can end up there, but due to the split time, can exist both in the Lands Between and the Dark Souls word.

But what if the Lands Between does exist in Dark Souls world? If we look at both game, they share a commonality in the Stone Dragon. Perhaps the Lands Between is simply a portion of Dark Souls land that the Stone Dragon, with their time power, cut away a portion of the land from Gwyn's order.

I figured it out! When Gwen removed Nameless King from the annul of history, he ended up in the Badlands and from there, ended up in the Lands Between, which is his conanically ending for where the Nameless King.

The Nameless King we met at Archdragon Peak is due to the convergence in time in DS3.
