Gmail has let you use multiple colors of stars and other icons to flag emails since 2015.

But even today, the Gmail iOS app will only display any star or icon of any sort as a Blue Star.
Meanwhile, iOS Mail and Outlook for iOS shows them all as a Red Flag.

Gmail has let you use multiple colors of stars and other icons to flag emails since 2015.
