It's a fun little game. Definitely worth a few play throughs and great for passing the time.

You'll have to mod your character skeleton manually. Can adjust muscle and height too. I know one of the plugins you can adjust the penis bones. I haven't played in awhile so I forget the name. I think you can also manually mod the mod but that's a bit more tricky.

My only recommendation is reaching out to sellers and asking for a custom.
