First place I saw the Dodecahedron Holocrons were in SWTOR, I had no idea they had existed before then, I only vaguely knew about the cubes and pyramid shaped ones.

Yeah, most of the angles in the show hide it, but in 'Lost in Translation', you can see a couple from above flying around the refinery. They're pretty far away from the camera though.

if we go by his Bio screen in Ent Season 4 he dies the night the 1701 launches.
Though I can't remember if that screen made it in the final episode or not.

Plus you'd sometimes see Iconia marked, a location that wasn't known to anyone until the 24th century.

they've done this a few times on maps, as their primary source in Star Charts and Stellar Cartography, which are maps set in the late 24th century. It was extremely apparent in some of episodes of Discovery when on some maps you could see they copied the path the Enterprise-A took in ST5

it is the Cardassian DMZ, the colour of the territory 'west' of it is the same colour they always use for Cardassian space.

it's not a lack of imagination, it's the art team copying maps made for the late 24th century and not altering details for the mid 23rd
