So much of my time after leaving the military and seeing corporate workforces is realizing how much of our “systems” are just people enforcing rules, with the inevitable breakdown when the enforcers no longer do their job or do it with partisan favors.
The usual engineering solution to “people are stupid” is to construct your systems to require the minimum of human intervention, but that doesn’t really scale to highly complex systems in which the components are also human beings.
