If the patient has poor functional status and has a high chance of deteriorating 2/2 difficulties presented post trach peg (poor rehabilitation potential, bed bound status, VAPs, skin breakdown, UTIs, protracted course that would set this state as a new baseline)

Another great reason to make ketamine access ubiquitous (easy to get from Pyxis in ICU/ED), my practice in the ICU is to have levophed infusing at a low dose at the time induction meds are being administered, which isn’t always quickly available for RSI. Ketamine would help a lot for that.
