Hank has already reached the outskirts of NV, so he's either lucky, or got a map. The Ghoul should be aware of local topography and will steer himself and Lucy to safety. Maximus however, is an idiot :)

We saw skulls at the end of the last episode. So that could either be a teaser of what's to come, or it could be a little glimpse into what has happened in the local area. Did the Deathclaws at the Quarry roamed as far as Camp McCarren and that why the NCR presence is reduced around the area of NV?
