This is the only time "Only A Poor Old Man" is said in the story, and it makes the final page so much more powerful.

Such as "A poor old man", "Only an old man", "Just a poor old man" and maybe more that I'm forgetting. It creates a sort of build up that pays off in the final page. When Donald calls Scrooge "Only a poor old man".

I believe the title is in integral part of why the final page works so well. We're presented with the phrase "only a poor old man" from the start, and throughout the story we hear many similar but not identical phrases.

It's the comic that mare me fall in love with Uncle Scrooge as a character. And as the first US comic, I love how it's titled "Only a Poor Old Man". It's incredibly ironic in a very epic way (as shown in the final page).

This is my favorite comic of all time, and it's really not a competition. No other comic I've read has ever come close to being as perfect of a story.
The last 5 pages (and one panel) of this story might be my favorite sequence in all of Barks' stories.
