Environmental factors would be seeking refuge more frequently for rest or ingesting strange materials to make up for poor mineral consumption

Too rainy? Rust
Too dry? Weakened frame and nanites
Too cold? Frame unable to bulk out due to lack of materials
Too hot? Coolant/energon loss

It would be both. Stress and poor energon intake would cause coding to start making changes in the frame

Inactive forge to delayed spark implantation. At one point too much material may be pulled from the frame for protoform development (illness to carrier or weak protoform)

Mammal scientists here so going to throw big words (I hate using google results but it is a lot)

But yes in mammals spontaneous fetal absorption can occur in several species (especially rabbits/kangaroos).

We also just joked around about if the forge can be removed and replaced later?
