What I’ve done in this sort of situation is slowly build a couple hours to myself into my day. Like, between work and getting home, you could find a nice spot to hang out and relax away from both responsibilities so you can get some rest.

I don’t know if you need to hear this today, but you can’t take care of your grandmother if you don’t take care of yourself. It’s not selfish, it’s just practical. Having a job to do 24/7 is an easy way to burn out and not be able to do anything as well as you want.
