💊 SGLT2i may protect kidneys during heart surgery!

🫀 Study of HF patients (LVEF >40%) on SGLT2i vs controls:
📈 36h post-op, eGFR was +11.8 mL/min in SGLT2i group (p=0.009)
❓ Current practice: stop SGLT2i pre-op — but is it needed?

🫀 Study compares two heart protection methods during surgery: Esmolol crystalloid cardioplegia (ECCP) vs. Calafiore blood cardioplegia (Cala).

💉 ECCP led to better heart function, flow, and lower damage markers after reperfusion in rat hearts.

🫀 POAF after cardiac surgery = red flag for future AF!

➡️ 24% developed POAF
➡️ POAF group had more paroxysmal AF at follow-up
⚡ ECG changes (P-wave duration, dispersion, amplitude) were more pronounced in POAF
🔍 These P-wave dynamics may help predict future AF risk

🫁 EVLP expands lung donor pool safely!

📊 In 23k+ US transplants (2011–2021):
✅ Same 5-yr survival with EVLP
⏳ Shorter wait at EVLP centers (48 vs 68 days)
⚠️ ECMO & Black race linked to lower survival

🔎 BITA vs. SITA in CABG

- 3,693 CABG patients from 2002–2012. PSM analysis + ROMA-like subgroup analysis.

✔️ Similar in-hospital mortality (1.8% SITA vs. 1.1% BITA)
✔️ BITA = Better long-term survival (15-yr: 64% vs. 51%, p < 0.001)
✔️ RMST shows growing benefit over time (+1.1 yrs at 15 yrs)

🚨🔍 Study finds key risk factors for longer hospital stays after aortic valve surgery!

🏥 Patients with prior valve surgery, endocarditis, COPD, or smaller valve implants face higher risks.

⚠️ Surgeons should aim for larger valves & personalized care.

🔍Investigated links between AADA surgery, weekdays, weather, and moon phases.

📊 Findings:
• More cases in winter ❄️
• Fewer surgeries on weekends 📉
• Higher mortality for weekend surgeries ⚠️
• No link to moon phases or weather 🌙🌦️

🫀💉 Post-op cardiac troponin 🚀 levels can complicate diagnosing type 5 MI after surgery.

🏥 This study explored links between troponin & LV mass index 🧪 but found no correlation—regardless of pre-op levels or surgery type.

📊 Study (10+ yrs) on 3,983 SAVR patients: Porcine valves showed better durability against late structural valve deterioration vs. pericardial valves but had higher rates of PPM, possibly impacting survival.

🫀 Valve choice depends on patient needs!

A single-center retrospective study (2010-2020) analyzed AKI after aortic valve replacement in AS + CAD patients:

✅ AKI rates were similar between TAVR+PCI & SAVR+CABG.
✅ No correlation between contrast volume & AKI.
✅ AKI didn’t impact short- or long-term mortality.

🫀 BMI & Minimally Invasive Aortic Valve Replacement
📊 Study finds obese patients have longer surgeries, more bleeding, but no higher mortality or stroke risk post-op.
⚖️ BMI may affect procedure complexity, but outcomes remain solid!
