Gaining experience does not undo those consequences. Characters advance, but they must live with what went wrong and deal with the results.

A character who misjudges a climb, survives the fall, and has to retreat is now closer to their next level than the one who never took the risk.
Over time, characters grow stronger because things went wrong and they adapted.
Failure is mechanically part of how characters level.
