Joyeuse — Yesterday at 10:44 pm ah! I know how to articulate something now: To build this abstract, broadest possible picture of my setting has required me to turn a blind eye to the particulars of the world I'll ask the players to know in my mini-campaign. It's really bugged me how disconnected I've felt, in doing that work, from the drama of the campaign, but perhaps now that that "abstract, bigger picture" work is more or less done...? ohhh I need to get the players invested in the places and peoples of the Yfel Republic. I need the players to care what happens to The Lights on the Coast

oh yeah for the longest time neutrois rep had been my white whale in every game featuring a character creator and portraying nudity I was really glad then to have been able to finally make that happen for at least one game with Baldur's Gate 3 (in Baldur's Gate 3's case they had the assets for it in the game the whole time and just never made them accessible to players - my mod more or less just enables a flag to make them accessible)

Excerpt from page 27 of AusPATH's History of Trans Health Care in Australia (AKA 'Report on Trans Healthcare History'): This was at the same time that the Commonwealth Government set up the Medicare public health system. The Minister for Health also commissioned Judge Robyn Layton to lead a review into the MBS. The aim of the review was both to simplify the MBS, as well as to determine which items should be covered by Medicare and at what rates. The review recommended against covering gender affirmation surgeries. The report laid out the justification: In the Committee’s view, gender reassignment surgery should only be performed on patients who attend special gender reassignment units. It believes that the best way to encourage this is to exclude the payment of Medicare benefits for such surgery and to provide funding for such units through Health Program Grants. (Citation 72) To this day, still most gender affirmation surgeries are still not covered by Medicare. At the time of the Layton Review there were three public clinics receiving state grant funding – two in Melbourne and one in Adelaide. Since 1988 there has only been the one: the Monash Gender Dysphoria Clinic. This has meant that only a miniscule number of trans people have had access to public subsidies for gender affirmation surgeries. The vast majority must pay out of pocket in the tens of thousands of dollars. (Citation 72: "Medicare Benefits Review Committee First Report,” (Canberra 1985), 220.)

On Reddit's transgenderau board: "Monash Gender Clinic - what surgeries have you had funded by them?" I'm aware that they suck. Has anyone gotten a surgery funded by them that wasn't top surgery or orchiectomy? I thought they funded vaginoplasty as well, but I heard from a GP that they won't? I ask because I'm trying to get phalloplasty funded by them, even though it's not currently something they do. I know it's unlikely, but I'm trying to find out how unlikely. I've been their the wait list for many years and it's more likely than me saving 30-200k any time soon so... It would also be cool to set a precedent & open the door. The idea back in the 80s was that Australia would have a UK-style system with gender clinic gatekeepers to funding, which was the (imo shitty) reasoning behind trans surgeries being excluded from medicare in the first place. Yet here we are in 2025 and there's only 1 gender clinic, and the funds they give out for surgery are very small overall.
